Personal Information
Note: we totally understand the situation: it's often hard to put together a portfolio of work, because all the work's behind firewalls or NDAs, etc. BUT it would be nice to see something – utility libraries, dot files, patches for open source projects, whatever. It's not a game breaker if you don't have this.
Programming language familiarity

Please check all of the programming languages or frameworks you're familiar with (where you'd be easily, and relatively quickly, able to create a program of at least 100-300 lines of code, based on your level of knowledge as it exists today.). We only use maybe a quarter of these on a day to day basis, so don't panic.

This also helps us with later interview stages. Even if you don't have a default language, put your favorite language, or the one you have the most experience with.
